Potential risks with the new antimigraine CGRP drugs

“CGRP (calcitonin gene-related peptide) is a short protein that mediates 2 major effects, at least that we think about in current biomedical conversations, ie, other effects might exist. CGRP-blocking drugs are new and thus we do not currently have enough experience to know the long-term risks in large numbers of patients. First, CGRP clearly plays a role in craniofacial pain—it is not the origin of the pain but is rather simply an intermediary between mitochondrial dysfunction at the starting end and pain and vasoconstriction on the resultant/manifest end. Second, CGRP is a potent vasodilator. The fact that the nervous system releases a potent vasodilator when confronted with an exacerbation of mitochondrial impairment obviously has survival value—the pain of #migraine is an unfortunate side-effect—and one that likely saves neurons from hypoxic stress and mitochondrial dysfunction. The reductionist pharmaceutical model of blocking CGRP in order to alleviate the pain it would otherwise induce is therefore not without clear and apparent risk—and the risk here is even more obvious (given that we already know that CGRP is a vasodilator) than was the increased risk of heart attack and stroke following treatment with Merck’s vasoconstricting rofecoxib/Vioxx, which was also designed to alleviate pain and inflammation and is estimated to have caused 140,000 heart attacks and 60,000 deaths before it was withdrawn in 2004. I am legitimately concerned that we are collectively walking into a minefield with these CGRP-blocking drugs because by blocking the vasodilatory effect of CGRP we are effectively promoting vasoconstriction, which carries risks of inducing hypertension, renal damage, heart attack, stroke, dementia, and more. In contrast and perhaps ironically, all of the migraine-alleviating nutritional treatments described in my books* have vasodilatory effects and provide greater clinical efficacy and efficiency at lower cost. Lastly, CGRP also plays important roles in skin/epithelial healing and also in gastrointestinal function, especially motility; the possibility of inducing clinically important alterations in healing and gut motility are of clear concern with CGRP inhibitors and –yet again– these problems are not seen with the time-proven nutritional interventions described in my migraine protocol.” Dr Alex Vasquez, September 2018

The migraine protocol excerpted from Inflammation Mastery (PDF cover, PDF contents) has been developed and published since Dr Vasquez was professor of Orthopedics and Rheumatology at Bastyr University nearly 20 years ago. Since then, the protocol has been repeatedly verified and updated, including a commissioned continuing medical education (CME) monograph published by Institute for Functional Medicine in 2008. Dr Vasquez has presented this information to international audiences of healthcare providers and has published related information in journals such as Nature Reviews Rheumatology and Journal of the American Osteopathic Association.
